SMS Terms & Conditions — Common Health Group Care Coordination Alerts
- Program description: The Common Health Group Care Coordination Alerts program sends informational text messages — such as appointment reminders, care coordination notifications, and connections to community resources — to patients and caregivers who are enrolled in a participating care coordination program and who have expressly opted in.
- Opt-in: You can enroll by completing the sign-up form at commonhealthgroup.org/sms.html and actively checking the consent box. Consent to receive text messages is not a condition of receiving any care, service, or purchase.
- Message frequency: Message frequency varies by care plan, typically 1–4 messages per month.
- Cost: Message and data rates may apply according to your mobile carrier plan. Common Health Group does not charge for the messages.
- Opt-out: Reply STOP to any message to cancel. After you send STOP, we will confirm your opt-out by text, and no further messages will be sent unless you re-enroll.
- Help: Reply HELP to any message, call (918) 901-9884, or email info@commonhealthgroup.org.
- Privacy: Mobile opt-in data will never be shared with or sold to third parties or lead generators. See our Privacy Policy.
- Carriers: Carriers are not liable for delayed or undelivered messages.
